Route 18 was formerly designated as Route S28, a prefixed spur of State Highway Route 28 through Middlesex and Monmouth Counties.

Where Route 16 turns south, the Post Road continues east as Northeast Main Street, which leads to the Uxbridge town line.

From Moncton, the highway continues southeast for to a junction at Aulac on the New Brunswick – Nova Scotia border ( near Sackville ) where the Trans-Canada Highway splits into the main route continuing to the nearby border with Nova Scotia as Route 2, and a route designated as Route 16 which runs east to the Confederation Bridge at Cape Jourimain.

Common usage, as well as signs until 1997, continues Route 128 east along the first 7 miles ( 11 km ) of Interstate 93 to the Braintree Split in Braintree, where I-93 turns north with Route 3 toward downtown Boston.

After it splits from I-95 in Peabody, Route 128 continues east into Cape Ann.
