The Sardinian archdeacon of Rome, Hilarius was elected bishop of Rome, probably on 17 November 461, and was consecrated on 19 November 461.

According to a letter to the Empress Pulcheria collected among the letters of Leo I, Hilarius apologized for not delivering to her the pope's letter after the synod, but owing to Dioscurus of Alexandria, who tried to hinder his going either to Rome or to Constantinople, he had great difficulty in making his escape in order to bring to the pontiff the news of the result of the council.

Hilarius erected several churches and other buildings in Rome, for which the Liber Pontificalis, the main source for information about Hilarius, praises him.

Matthisen speculates that he may have been a relative or " even the son " of the Hilarius who had been prefect of Gaul in 396 and of Rome in 408.

There is mention of a gift of these two vessels, both of gold and adorned with 250 white pearls, that the Emperor Constantine gave to Saint Peter ’ s Basilica in Rome, and of silver towers and golden doves given to particular churches by Pope Innocent I and Pope Hilarius.
