To cover the Part A and Part B benefits, Medicare offers a choice between an open-network single payer health care plan ( traditional Medicare ) and a network plan ( Medicare Advantage, or Medicare Part C ), where the federal government pays for private health coverage.

Medicare Part D covers outpatient prescription drugs exclusively through private plans, either standalone prescription drug plans or through Medicare Advantage plans that offer prescription drugs

Initially Medicare consisted exclusively of Part A, which covers hospital and other inpatient services, and Part B, which covers outpatient care, physician visits, and other “ medically necessary services .” Congress then added Medicare Part C ( originally called Medicare + Choice, then later changed to Medicare Advantage ), which allows enrollees to receive their Medicare benefits through a private plan, under the Balanced Budget Act of 1997, while Medicare Part D was created under the Medicare Modernization Act of 2003.

Fares are $ 1. 50 for adults, and $. 75 for children 5-11, for students with valid I. D, for handicapped / Medicare recipients, and for senior citizens age 65 and up.

Parts B and D are funded by premiums paid by Medicare enrollees and general fund revenue.

Medicare Part D covers prescription drugs.

Medicare Advantage plans, also known as Medicare Part C, are another way for beneficiaries to receive their Part A, B and D benefits.

Medicare Advantage Plans that also include Part D prescription drug benefits are known as a Medicare Advantage Prescription Drug plan or a MA-PD.

A third of beneficiaries with Part D coverage are enrolled in a Medicare Advantage plan.

Anyone with Part A or B is eligible for Part D. It was made possible by the passage of the Medicare Modernization Act.

Unlike Original Medicare ( Part A and B ), Part D coverage is not standardized.

It should be noted again for beneficiaries who are dual-eligible ( Medicare and Medicaid eligible ) Medicaid may pay for drugs not covered by part D of Medicare, such as benzodiazepines, and other restricted controlled substances.

Medigap policies sold after the introduction of Medicare Part D on January 1, 2006 are prohibited from covering drugs.

Three of these graphs are shown below, and they clearly show why Medicare ( as currently formulated ) is on the path to fiscal insolvency: No matter what the wage level, marital status, or retirement date, a man or woman can expect to receive benefits that will cost the system far more than the taxes he or she paid into the system.

Due to a " spousal benefit " built into the Medicare formula, the married person will get net benefits ranging from $ 393, 000 to $ 525, 000.

Medicare came into effect on 1 February 1984, following the passage in September 1983 of the Health Legislation Amendment Act 1983, including amendments to the Health Insurance Act 1973, the National Health Act 1953 and the Health Insurance Commission Act 1973.

The proposal would keep traditional Medicare as an option, though it would also introduce private health insurance companies into an exchange in which they would offer competing plans to be paid for with government vouchers.

The responsibility for enrolling beneficiaries into Medicare and processing premium payments remained with SSA.

The costs of these provisions are offset by a variety of taxes, fees, and cost-saving measures, such as new Medicare taxes for those in high-income brackets, taxes on indoor tanning, cuts to the Medicare Advantage program in favor of traditional Medicare, and fees on medical devices and pharmaceutical companies ; there is also a tax penalty for those who do not obtain health insurance, unless they are exempt due to low income or other reasons.

In 2009, the U. S. Centers for Medicare and Medicaid Services announced that it would begin using ICD-10 on April 1, 2010, with full compliance by all involved parties by 2013.

In 1983 Reagan instituted a payroll tax on Social Security and Medicare hospital insurance.

These include Social Security and Medicare taxes imposed on both employers and employees, at a combined rate of 15. 3 % ( 13. 3 % for 2011 ).

Johnson was greatly supported by the Democratic Party and as President, he was responsible for designing the " Great Society " legislation that included laws that upheld civil rights, public broadcasting, Medicare, Medicaid, environmental protection, aid to education, and his " War on Poverty.

The Great Society program, with its name coined from one of Johnson's speeches, became Johnson's agenda for Congress in January 1965: aid to education, attack on disease, Medicare, Medicaid, urban renewal, beautification, conservation, development of depressed regions, a wide-scale fight against poverty, control and prevention of crime, and removal of obstacles to the right to vote.

The Medicare program was established on July 30, 1965, to offer cheaper medical services to the elderly, today covering tens of millions of Americans.

When Johnson proposed the Great Society in 1964 – 65, Dole voted against some War on Poverty measures like public-housing subsidies and Medicare, thus appealing to conservatives.

These out-of-pocket costs vary depending on the amount of health care a Medicare enrollee needs.

Medicare also spurred the integration of thousands of waiting rooms, hospital floors, and physician practices by making payments to health care providers conditional on desegregation.
