The encounter between Dr Prunesquallor and the Countess Gertrude when she demands goat's milk, and the subsequent attempts to procure it for her, also provide an element of comedy.

Steerpike uses his charm and fast tongue to insinuate himself with the castle's physician Dr Prunesquallor, and acts for a time as his apprentice.

This is overheard by the castle's doctor, Dr Prunesquallor, who is greatly disturbed to hear it.

Dr Prunesquallor discusses Steerpike's words with the Countess Gertrude, but they disagree over its meaning and the ambiguity over exactly what Steerpike meant is never resolved.

She also has an enduring friendship with Dr. Alfred Prunesquallor, the Gormenghast physician.

Fuchsia was buried on Gormenghast Mountain, at a site overlooking the Castle chosen by her mother, in a ceremony at which neither her brother the Earl, recovering as he was from the severe fever that followed his killing of Steerpike, nor Dr. Prunesquallor, who was caring for the Earl, could attend.

A little later, Steerpike accompanies Fuchsia to the house of Dr. Prunesquallor, and becomes his apprentice for a while.

Dr. Alfred Prunesquallor is the castle's resident physician.

Despite this, both the Countess and Dr Prunesquallor are disturbed and uneasy about all that has happened, and disturbed about Steerpike's sudden rise.
